Agreement

The undersigned Property Owner(s) (“Owner”) hereby acknowledge that property owned by Owner at the address listed above is presently connected to and being served by the sanitary sewer system operated by the Jordan Basin Improvement District (“District”). Owner understands that pursuant to the District’s Rules and Regulations, Owner is ultimately responsible to pay fees and charges for sewer service to the above listed property.

Owner hereby requests the District to send the District’s billing statements for sewer services rendered to the property to the Renter listed above at the above stated property address. Accordingly, Owner understands that Owner will not receive billing statements. Notwithstanding the foregoing, the District reserves the right at any time to bill Owner direct for sewer service provided to Owner’s property. In the event that Renter fails to pay any sewer charges or fees billed by the District, Owner will promptly pay the unpaid balance in full. Owner hereby agrees to notify the District of any tenant changes.
